How does the health system work in Indonesia?

Indonesia’s public health system is undergoing significant changes. Although it is still in a not very favorable position compared to other countries, Indonesia’s facilities are gradually improving. In addition, the government is committed to improving its investment in health services.

 

For the most part, public health facilities in Indonesia are modest. Big cities like Jakarta have good public hospitals, but rural clinics have very limited resources. In addition, most public facilities need investment in modern equipment, laboratory services and staffing resources.

Private facilities, in contrast, are clean, well-organized, and up-to-date. A much higher percentage of staff speak English compared to public centers. Also, waiting times are much shorter and there is a good range of specialists available. Patients can expect to receive care in a comfortable and private environment.

 

Do expats need to have health insurance in Indonesia?

Simply put: yes! Health care in Indonesia is mandatory for expatriates to have a resident visa or work permit.

So, it’s essential that you insure yourself with one of the coverages I’ve selected from the list below 🙂

But in addition to being mandatory, an international health plan gives you more peace of mind. Especially being in a new country, different from your own, with laws, structures and a language you may not know, and unpredictable healthcare costs.

So, by securing international health insurance coverage you will be assured that you are covered in any emergency or unexpected situation. In addition, there are several other benefits that you can get with an international plan (as I will say in the next question).

As an immigrant in Indonesia, should you get an international health plan like the ones recommended in the list below? Or should you get a national plan?

So, this is a choice that will depend on you, as only you know your specific medical needs, the specific coverage you will need in your insurance, from the duration of your stay in Indonesia to the options available from national plans.

 

An international health insurance plan must meet your in-country coverage requirements. If you choose to get an international plan, be sure to check with Turkish government embassies to find out if there is and what is the minimum insurance coverage required.

I’ve never lived in Indonesia so I don’t know enough about the national plans that might be available and so I can’t say much about them.

With that in mind, below are some of the reasons you might want to choose an international health insurance plan over public coverage:

  • Compared to the costs you may incur, international health insurance may have cheaper monthly fees;
  • International health insurance may be easier to use if your stay is short in Indonesia, as you can choose exactly how long you want your plan to be;
  • International health insurance has greater flexibility as you can cancel or extend it at any time;
  • International health insurance provides coverage in other countries, not just Indonesia (hence “international” health insurance).

It is important to know that travel insurance is not ideal for immigrants. Although it is possible to have travel insurance for longer periods, it is only for travelers with temporary stays in a country. Travel insurance covers are not valid for someone who will stay in another country for the long term. 

In addition, travel insurance will not meet your health insurance coverage required by law to live in Indonesia.

You will see that they have the possibility of plans with long stays of 1 year or more, but also short-term travel plans.

Their website is intuitive and easy to navigate, with filters for dates, plan duration, destination and more that make your search easier. In addition to these filters, you can add your personal details like age and specific needs for the best coverage. 

Thus, IMG will provide a detailed quotation that allows a comparison of plan and coverage options for you to have freedom and choose what is best. 

Only pay attention to the details of each plan, as some do not have coverage for all age groups and not all of your plans cover the needs of Covid-19.

One of the advantages of SafetyWing is that its policies are a little more accessible and easier to understand than the other options on the market. SafetyWing also covers children aged 2-10 for free if their caretakers have a plan with them.

 

Each plan has a specific price that depends on your age. The cheapest SafetyWing plan doesn’t cover pre-existing conditions, but it does have coverage of up to 250,000 USD. Though you do have the freedom to add additional pre-existing condition coverage separately if you need to.

What I like most about SafetyWing is that they offer 30 days of coverage for free in your home country for every 90 days that you use it overseas, like in Japão or the EAU in general.

For anyone who is moving to Japão, I recommend the Diplomat long Term or the Diplomat International Plan. Both have very complete medical coverage that in general includes all that an expat may need in their new home.

Types of Indonesian Visas

The types of visa most used by people who want to live in Indonesia are:

 

Limited stay permit

Expats intending to stay in Indonesia for longer than covered by a tourist visa will need to obtain a Limited Stay Permit, also called an ITAS (Izin Tinggal Terbatas). O
ITAS is valid for six to 12 months for foreigners employed in
Indonesia. An ITAS can be renewed for the same period it was initially granted. This can be done a total of up to four times.

 

Limited stay permits are granted to:

Foreigners entering Indonesian territory on a limited stay visa or foreigners receiving a change of status from a Visiting Permit to Stay, which includes:

  • Foreigners in the scope of investment;
  • I work like an expert;
  • Performance of duties as clerics;
  • Participate in education and training;
  • Conduct scientific research;
  • Join the husband or wife who holds a limited stay permit;
  • Join the father and/or mother of children of foreign nationality who have a family
  • relationship with the father and/or mother of Indonesian citizens;
  • Accompany father and/or mother holders of limited stay authorization or permanent stay authorization for children under 18 (eighteen) years of age and unmarried;
  • Foreign nationals who are former Indonesian citizens;
  • Foreign elderly tourists.
  • Children whose father and/or mother have a limited stay permit at the time of birth in Indonesian Territory;
  • Master, ship crew or foreign experts on ships, floating equipment or installations operating in Indonesian territorial waters and jurisdiction in accordance with the provisions of laws and regulations;
  • Foreigners legally married to Indonesian citizens; or
  • Children of foreigners legally married to Indonesian citizens.
  • Limited stay permits can also be granted to foreigners to do short-term work.

 

Permanent residence permit

After an expatriate has been in Indonesia for five years on a limited stay permit, they are eligible to apply for a permanent stay permit, also known as KITAP (Kartu Izin Tinggal Tetap). If expatriates are married to an Indonesian citizen, they can apply for KITAP after just two years.

Only the following people are eligible for a KITAP:

1. Foreigners who are married to an Indonesian

2. An Indonesian company may have foreign investors, directors or commissioners (PT PMA)

3. Retirees from other countries who want to live in Indonesia

4. Citizenship is being regained by Indonesians.

 

The following are the required documents for KITAP:

1. Obtaining a passport (active for at least 18 months).
2. KITS.
3. a note from the sponsor
4. Your sponsor’s Indonesian ID (KTP).
5. The main number for tax obligations is (NPWP).
6. In your statement letter you said you had no plans to work in Indonesia (for retirees).
7. Work permit (IMTA) and legal paperwork from the sponsoring company (for foreign investors).

The application procedure takes about three months.
